Angelica Campa  

Resident Service Coordinator

Ms. Campa is the Senior Service Coordinator at Las Palmas’ Indio Gardens Senior Apartments. She is an integral part to the onsite management  team.  In order to support the resident’s independence and overall well being, Ms. Campa provides extensive case management, advocacy, outreach and linkages to resources and organizations within the community.

Ms. Campa holds a Bachelor of Arts degree in Psychology and Family Studies and Human Development. Ms. Campa has been working in social services for over six years with an emphasis on elderly for the past four years. Her background ranges from assessing and working with families in their homes to placing seniors in appropriate eldercare placements.  In addition, Ms. Campa is fluent in Spanish and English.

Karen Denicore

Resident Service Coordinator

Ms. Denicore provides services and resources to residents at Baywood Senior Apartments in Oakland California. She is an experienced service coordinator and case manager with over twenty years of experience providing services and managing programs for non-profit, state, and local government agencies.

As a graduate of the UC Berkeley School of Social Welfare, Ms. Denicore started out in social services with a focus on transitional age youth and later moved on to specialize in workforce development. With employment experience in government, healthcare, and non-profit agencies, Ms. Denicore discovered the perfect match for her work experience, passion for advocacy, and wily negotiation skills in providing services to seniors starting in 2005. She is very pleased to be providing services to seniors in her own community and looks forward to continuing and building on the strong service coordinator program at Baywood Apartments.

Colleen Keegan Falcone

Resident Service Coordinator

Ms. Falcone provides service coordination and resources to residents at Bethlehem Tower in Santa Rosa California.   She has many years of experience working for non-profit organizations, and working with the local state and federal agencies.    She was the Executive Director for a major non-profit organization which provided Adult Daycare, Respite, Financial Assistance, Safety Assessments, Environmental Home Modifications and Outreach services to the elderly, disabled and economically disadvantaged.  Over the years, Ms. Falcone has been invited to present at annual senior care conferences, lecturing on Environmental Home Modifications and Medicaid based senior care programs. Additionally she has represented several non-profit agencies at the annual state legislative session, advocating against budget cuts to government funded senior programs.   Since the age of 16, Ms. Falcone has been an active volunteer in her community.  In the past she was a state Ombudsman, and currently she volunteers with the Friendly Visitor Program through Petaluma People Services.

Vicki Fernandez

Resident Service Coordinator

Ms. Fernandez supervises social services at Las Palmas ‘s Summercrest community. Ms. Fernandez joined the National City Collaborative in May 2003 and has made outstanding contributions to the Summercrest residents’ lives and environment, promoting incredible positive change.  She has developed a number of noteworthy service enriched programs, including, but not limited to: computer education classes; daily lunch program; on-site mental health services, including individual and marriage therapy; medical and nutrition program; exercise classes; and senior social hour.   These programs empower residents, support self-sufficiency and promote a sense of well being.

A Graduate of Arellano University, Ms. Fernandez holds a Master’s Degree in Psychology and a Bachelor of Science Degree in Education from Philippine Normal University. She was a member of the High Honors Society. Her experience ranges from teaching elementary and secondary school to pharmaceutical and nutritional work.

Ms Fernandez is the recipient of the National HUD Neighborhood Networks’ Catalyst for Community Change Award. This award honors Ms. Fernandez, whose dedicated commitment and outreach efforts have brought about measurable change at Summercrest Apartments.

Paula Padelford, RN, PHN

Resident Service Coordinator

Ms. Padelford is the Senior Service Coordinator at Las Palmas’ Palm Springs Pointe and has been working in social services for over 15 years.  Mrs. Padelford is a vital part of The John Stewart Company management onsite team. She provides extensive case management, advocacy, outreach and linkages to resources and organization within the community which help increase the likelihood that the residents will remain independent and healthy.

Ms. Padelford holds a Bachelor of Science degree in Nursing from Point Loma Nazarene College. Ms. Padelford is a Registered Nurse and her experience ranges from running support groups for senior caregivers to providing case management for at-risk seniors. She is a member of the organizing committee for the start-up Southern California chapter of the American Association of Service Coordinators (AASC).

Elsa Rodriguez

Resident Service  Coordinator

Mrs. Rodriguez provides social services to senior residents at  Las Palmas’s Summercrest community in National City, California.  She has been a part of the Summercrest team  since July 2007 and has been an asset to the community, providing excellent standard  of care.  As the Senior Service  Coordinator II, Mrs. Rodriguez provides intakes, assessments, on-going case  management and referral services to seniors and adults with  disabilities.  She coordinates  educational sessions for residents and monitors the ongoing services that are  delivered by community providers.  Mrs.  Rodriguez assures that the programs are appropriate and that the residents are  receiving quality services and resources.   This outstanding care empowers the residents to be active, aware, and  self sufficient.

Fluent in English and Spanish, Mrs. Rodriguez has an  extensive background in social work. She was employed at Rady’s Children’s  Hospital, San Diego for thirteen years and specialized in the maltreatment of  children and families.  In addition, Mrs.  Rodriguez was a drug counselor for two years at a drug rehabilitation clinic.

Beda Gaona

Resident Service Coordinator Aid

Ms. Gaona has a background in child development, art, and computer science.  She has worked at Summercrest Community Center since 2006 as a  Resident Service Coordinator Aid.  Ms. Gaona Ms. Gaona also manages the volunteer program and free bread and milk distribution to qualifying residents.  Fluent in English and Spanish, Ms. Gaona engages residents and members of the community to provide unique and impactful social services.
